Tension pneumothorax is a condition where due to a new hole in your chest and lungs by gun shot or shrapnel, when you breathe in normally your diaphragm depresses down creating negative pressure in your chest allowing your lungs to expand, but now air is also able to go out through the lung into the chest through that new hole, and is unable to enter back in the lungs or escape out your chest, filling the chest cavity with air compressing the lungs restricting their ability to expand. This is a medical emergency. A needle decompression is a relatively simple life saving procedure where a large bore needle is inserted into the chest allowing air a place to escape, a very reassuring hiss sound of escaping air is emitted when performed correctly, which is what the medic is listening for around 00:38. The needle can be seen at 00:37 before being hidden. The catheter remaining in the chest can be seen at 00:40 held between the medics thumb and index finger.
