Looks like a pair of giant old bull testicles or animal hearts they planted in there to give the impression of it being real..doubt anyone is gonna be strong enough to just reach in and pull stuff out. Kinda like those Indian "healing" doctors did..held some meat in their hands tightly and pretended to be super duper healers years back by making out they pulled it out of some poor schmuck.
