Cactus Road Critical Incident Briefing | Desert Horizon Precinct | Officer Michael Uglade-Flores #11219 On May 25, 2025, at approximately 2:30 p.m., a Desert Horizon Precinct patrol officer observed 46-year-old Sam Perone operating a vehicle on East Cactus Road in violation of Arizona traffic statutes — specifically, traveling at 30 mph in a posted 40 mph zone. The officer initiated a lawful traffic stop, and Perone's vehicle came to rest near the intersection of 25th Place and Cactus Road in north Phoenix. Before the officer could fully bring his marked patrol unit to a stop, Perone had already exited his vehicle and assumed a confrontational posture. Upon approach, the officer calmly explained the reason for the stop and requested Perone's driver's license. Perone refused to comply. Over the next several minutes, Perone escalated from verbal non-compliance to aggressive verbal assault — repeatedly cursing at the officer, demanding a supervisor be dispatched, questioning why he was being "followed," and advancing aggressively toward the officer despite multiple commands to step back. The officer requested additional units via radio, describing the subject as "unhappy." The confrontation transitioned from verbal to physical when Perone struck at the officer's head, initiating a full physical altercation. A bystander in a black T-shirt, who had been in the area across the street, intervened and attempted to pull Perone away from the officer — consistent with Good Samaritan conduct. Despite this intervention, Perone continued to assault the officer, at one point bear-hugging him, driving his head downward, and striking it with his knee. The physical struggle lasted approximately 30 to 50 seconds. During the altercation, the officer's body-worn camera was dislodged and fell to the ground. The device continued to record audio, capturing the sounds of the ongoing struggle.
