Looks like a recent open heart surgery patient who split the incision open. That's why they give you a pillow to put over your chest when you cough. Maybe they didn't use titanium thread to sew him back up.

Yeah, I have the scar to commiserate...

The Docs might have gone back and successfully sewn him back together, but the lack of a sterile field with an open chest probably killed him in the days to follow....
